Mark has just shown Jesus healing Simon's mother-in-law and then many sick and demon-possessed people brought to him after sunset, since the Sabbath had ended and carrying the sick was permitted. The crowd gathers at the door of Simon and Andrew's house in Capernaum, a small fishing town, so "the whole town" is a vivid hyperbole for a remarkable turnout.
