At once Jesus knew in His spirit that they were thinking this way within themselves. “Why are you thinking these things in your hearts?” He asked.

The scribes have just heard Jesus tell the paralytic his sins are forgiven, and in their hearts they accuse him of blasphemy, since only God can forgive sins. Jesus perceives their silent reasoning, not through anyone telling him but by knowing in his spirit.
