But if anyone causes one of these little ones who believe in Me to stumble, it would be better for him to have a large millstone hung around his neck and to be thrown into the sea.

Jesus is speaking to the Twelve in Capernaum, continuing the lesson begun when He set a child among them (verses 36 and 37). The "little ones who believe in Me" are likely both literal children and the humble, lowly disciples He has just held up as an example.
